I certify that I am the parent or legal guardian of the camper and grant permission for my son or daughter to attend the Matt Lottich Life Skills Basketball Camp session(s) selected above. I have no knowledge of any physical impairment that would affect the camper’s participation in the camp program and affirm that the registration information provided is accurate. In the event that emergency medical treatment is needed when neither parent can be contacted without risk or delay, I authorize MLLS to assume responsibility for acting in the best interest of the camper. I hereby waive and release Paye’s Place, MLLS and its staff members from any liability for any injuries or losses incurred during the event or when traveling to or from the event. I also give full permission for MLLS to utilize any photographs of the camper for publicity and advertising purposes.

The camper agrees to refrain from foul language, unnecessary roughness and any other dangerous or foul play. The camper understands that failure to adhere to these policies, along with general misconduct, are grounds for immediate dismissal. The camper also agrees to play hard and have fun while at camp.
